Hi, I've been taking Warfarin since January and I've had a tooth extraction this afternoon. I had an INR reading this morning and was told by the anticogulant nurse that my reading of 3.8 was okay - below 4.0 should be acceptable by the dental surgeon, who confirmed this and the extraction went ahead. I left the pad over the cavity for an hour and it has now been 4 hours since I had the tooth out and everything is okay. Bleeding has stopped. There are lots of conflicting messages on this link, but I thought I'd post my experience on here anyway.

Had upper back tooth removed that was not planned, but last day cobra ins was in effect,so decided to have extraction. This was 4 days ago & still having bleeding off & on.Using tea bag to help with the bleeding. How long can this keep going on before i should be concerned.I am on coumadin & right now also taking lovenox shots due to sophisticated blood work i had last week.Dr.wanted to test me to see if i will have to be on coumadin from now on.Had dvts in legs & lungs last year after back surgery. Thanks for any input.

I'm a dental hygienist and I'm also on warfarin :-)

Make sure the dentist knows you are on anti-coags and he will OK it with the prescribing doctor to remove you from them for approx 3 days prior to the extraction, and he may want your INR done first too. If you're on warfarin for a clot, your doc may not want you off the warfarin yet, I'm not sure if your extraction can wait but you need to work it out with the dentist and the doc that prescribes your warfarin. ---About 4 years ago a patient came in and had about 4 teeth pulled and didn't tell one of the dentist's in my office he was on anti-coags and he died as a result! His INR must have been off the chart because the sockets never clotted, they just kept bleeding and bleeding and he never went to the OR or called the dental office, YIKES!

Ive had 5 extractions on Warfarin and as long as your INR is under 2.5, then its fine.

Mike just make sure the dentist knows you are on anticoagulants and let him/her decide what is a safe INR and if he/she needs to coordinate with your doctor to take you off for a few days or not. They will make the right decision they deal with this all the time.

I just had 9 upper teeth removed. I stopped my meds for 2 days and was at 1.7. My oral surgeon feels anything at 2 or under is fine. My bleeding stopped the same night I had the teeth removed.

For those of you with mechanical heart valves, you should not stop your warfarin dosage. Under no circumstances should you stop warfarin for 5 days with a mechanical heart valve. I wouldn't even be comfortable stopping it for 1 day, and this was confirmed by my cardiology team. There are lots of oral surgeons who will do the extraction at the low end of the therapeutic range of INR, i.e., 2.5. The risk of complications related to clotting around the valve is far greater (and more serious) than the bleeding risk in the mouth.

I'm on warfarin after having my aortic & mitral valves replaced by metal valves 6yrs. ago & now due to an infection have to have them replaced again which is easier than getting my tooth extracted. Dentist wants me off warfarin for 5 days, cardiologist wants to lower my INR for 2 days & my G.P. who I see every week is up in arms because my INR has never been under control. And I'm in the middle with sever pain shooting along my jaw!!

I have been on warfarin since 2006. I have a real bad infected tooth that needs to come out now ,but because I do take warfarin it has been a slow process. I had to start taking lovenox shots 2 times a day. Now I have blood blister on my legs. The antibod is not working so it was changed. I am suposse to have it out Thursday. sometimes the cure is worst then the disease.

I had my tooth removed 29th Feb. finally.I was scheduled to have it removed the day before but when I turned up the dentist refused to do it because of the hassle with antibiotics & warfarin so booked me in 4 the next day with another dentist who had experience with these problems but first he wanted to know what my INR was. Test put this dentist in doubt, my INR was 3.5. After some deliberation he decided to do it, reluctantly. 3 nurses & 2 dentist crowded into the room & began. I only had a little more bleeding because of the warfarin but an hr. later I caught the bus home! A small amount of bleeding for 2 days, the worst part was the pain in the next tooth because of all the pressure that was applied. 1 wk. later & the gum is great but the next tooth is still a little painful. Glad of all the preparation because my gum had to be cut to get the tooth out but I didn't need any special care afterwards. All that worry & stress was a waste of time in the end!

I am having 2 teeth out tomorrow. One has a broken root under the crown. It's a molar. The other is the opposing tooth at the top. My oral surgeon says my INR has to be under 3.0. He says no problem. Might bleed a little longer, but was very confident all will be well without having to worry about bridging me, etc. I hate Lovenox. Makes me too anxious. We'll see how it goes in the morning! Love oral surgeons!

I had a tooth out and my INR was 2.4.

In fairness to the Dentist he did say anything over 3 he would not have extracted. I just sat in the chair for about ten mins until he was happy that a clot had formed as I was biting on a gauze to stem the bleeding,,All ok and no extra charges,

Hope this can be of some assistance,

Chris In Bonnie Scotland.
